It's the day after the prom and everyone is worried about Izzie, who is still mourning Denny. She just lies on the bathroom floor of the house and refuses to eat or change out of her prom dress.
Cristina tells Meredith she's the best candidate to go comfort Izzie, because she's "dark and twisty."
Miranda stays with Denny's body until the funeral home picks him up. There won't be a funeral, at his family's request. Before he's wheeled off, she tells him, "I'm sorry. I'm so sorry."
Meredith confesses to Cristina that she snuck off from the prom and slept with Derek, but that she has no idea where things stand between them.
Adele makes an appointment with Richard, saying it's the only way to see him. She offers an ultimatum: Retire or she's leaving him.
Omar and his wife, Giselle, come into the E.R. She was driving them both to the hospital for severe flu when she passed out at the wheel. Omar is running a high fever and it turns out he was exposed to someone with bubonic plague. Quarantine is immediately declared.
Derek and George are on their way to Meredith's house when they are stopped by health officials: they just operated on Giselle and now both of them have to stay in quarantine, alone, in the locker room. They end up discussing the women in their lives. George says he's going to tell Callie he loves her "soonish," and Derek says he shouldn't wait. George asks if he ever told Meredith he loved her, and Derek says no. George reveals he said, "I love you," to Meredith and that went so badly that that's why he can't bring himself to say it to Callie.
Alex takes charge of a newborn baby that was abandoned in a trash can. It has a serious blood condition that requires a transfusion from the mother, if they can find her. The only likely suspects are a troop of 14-year-old Girl Scouts, who all deny it's theirs. As Addison tries to narrow down who the mother is, she recalls the night that Derek found her with Mark and how she begged him not to leave. "The truth always comes out," she tells the last two girls who might be the mother, and one finally confesses.
In other flashbacks, we see how the interns first met at a mixer: Alex tells Izzie she won't make it through the first year; Cristina gushes to Burke how much she admires him; George tries to chat up a disinterested Meredith. Derek recalls meeting Meredith at Joe's bar over tequila and Scotch.
Omar is in quarantine when Miranda stops to check on him. Because the health officials won't let her break the seal, she has to tell Omar through the door that his wife has died. He starts to hyperventilate and she talks him through it, telling him she believes in an afterlife, and that it's beautiful.
Callie shows up at the house with food and starts cooking. She reassures Meredith she won't tell anyone about walking in on her and Derek. Finn shows up to talk to Meredith, which is the moment she decides she needs to talk to Izzie. Finn tells Meredith that he doesn't want to know what happened with Derek, but that he's going to give her time to decide, but that Derek isn't good for her.
Derek and George are finally released from quarantine. As Derek watches, Callie tells George she was so worried and that she really, really loves him. George says nothing.
Derek finds Meredith and finally says that he's in love with her, and that she has a choice to make. And that when he had the same choice, he made the wrong one.
Cristina tries to help Izzie by telling her about shiva, the Jewish grieving ritual for the dead, and Izzie asks her how she'd feel if she ever lost Burke. Later, Cristina goes to Burke's hospital room and unexpectedly starts sobbing. "Don't ever die," she tells him.
Finally, with Meredith beside her, Izzie stands up and decides she's ready to take off her prom dress.

George: My point is you¡ve lived. If you die who cares?. If I die, what? This is it? Callie told me she loved me, I just sat there. I mean, I wasn¡t ready, but right now I¡m gonna die and I ain¡t getting a chance to say I love you back.
Derek: Do you? Do you love her?
George: Maybe. Eventually. I could. One day... soonish.
Derek: Soonish? I will love you soonish?
He laughs
George: You ever told Meredith? You ever told her that you love her?

George: I did, but I¡m with Callie. It¡s different, but...
Derek: You should tell her. Even if it¡s soonish. You should tell her before it¡s too late.
George: You mean before I die of the plague?
Derek: Before somebody else comes along.
(voiceover at beginning of episode)

Meredith: I'm always sorry in the morning. But tomorrow is my first day at work, so keep 'em coming.
Derek: Double scotch, single malt please. (to Meredith) So, is this a good place to hang out?
Meredith: I wouldn't know, never been here before.
Derek: You know what? I haven't either. First time here. I'm new in town. Never been to Seattle. New job soon. (Meredith doesn¡Çt say anything) Ah, you're ignoring me.
Meredith: Um, trying to.
Derek: Well you shouldn't ignore me.
Meredith: Why not?
Derek: Because I'm someone you need to get to know to love.
Meredith: Really?
Derek: Oh, yes.
Meredith: So if I know you, I'll love you?
Derek: Oh, yes.

Derek: Just hiding my pain. (they laugh) So what's your story?
Meredith: I don't have a story. I'm just a girl in a bar.

(During Addison¡Çs flashback)
Addison (while Derek is gathering her clothes up): Derek, Derek, listen to me--- Derek, you can't do this, Derek. Derek, we have to, we have to talk about this.
Derek: No we don¡Çt.
Addison: You have to give me a chance to explain. (starts walking down the stairs with her clothes) What are you doing? What are you doing with my clothes, Derek?! It was one time. I know that's what people say. I know that is what always gets said. It's just I don't even know how it happened, I don't know what I was thinking. It was just he was just here, it was just...
Derek: You screw my best friend and all you have to say is, 'He was just here.' (throws Addison's clothes outside) Get out.
Addison: No.
Derek: Get out!
Addison: No, no I'm not going. We have to talk about this.
Derek: Get out of my house now!
Addison (sits down on the stairs): No, I'm holding my ground because we don't quit. We have to work this out.
Derek: Get out. (he grabs Addison¡Çs arm and starts pulling her towards the door)
Addison: What are you doing? Derek? (Derek shuts the door on Addison. She places her hands on the door, and he leans against it and places his hand on the opposite side) No, no. (Addison starts crying as she is out in the rain) Please, Derek, please. Please. (Derek opens the door, letting her in) I'm sorry. I'm sorry. You have to give me a chance. (they hug) You have to give me a chance to show you how sorry I am. Okay? Okay?
Derek: Okay. (Derek shuts the door) I'm gonna go, you stay. I'll get my clothes in the morning.
Addison: No, no, no, no, no, no. We can survive this, Derek. We can survive this. We're-- we're Addison and Derek.
Derek: I can't look at you. I look at you and I feel nauseous. It's just...wha---wha? (shakes his head) We're not Derek and Addison anymore.
Addison: If you go now. if you go now we are not gonna get through this. If you go now, we don't have a chance. We don't have a chance, if you go now, if you go... (Derek walks out the door)
(Izzie is still in the bathroom)
George: Okay, she¡Çs been in there all night. We have to do something.
(Alex, Cristina, and George all turn and look at Meredith)
Meredith: Why are you all looking at me?
Cristina: Because this is familiar territory for you.
Meredith: There is nothing familiar about this. Unfamiliar. Denny died. The man she loves died.
Cristina: Yeah, but you¡Çre all dark and twisty inside.
Meredith: Dark and twisty?
Cristina: What, with the Alzheimer¡Çs thing and the father you don't talk to.
Alex: And the tequila thing, and the inappropriate men thing.
George: You are dark and twisty inside, Meredith, and now Izzie is dark and twisty inside.
Meredith: So all of a sudden I¡Çm the president for people with crappy lives?
George: What if the results come back and she has the plaque? Then what? We die in here with the crappy benches and the dirty scrubs and the lockers fill of old food.
Derek: There¡Çs food in the lockers? (Derek gets up, and opens a locker)
George: Dr. Shepherd, those are people¡Çs personal---that¡Çs Alex¡Çs apple. (Derek takes a bite) That¡Çs just rude.

Derek: Do you? Do you love her?
George: Maybe. Eventually, I could. Someday....soonish.
Derek: Soonish. I love you soonish?
George: You ever tell Meredith? You ever tell her that you love her?
Derek: No.
(During Izzie¡Çs flashback)
Alex: Surgery's hard core.
Izzie: I'm hard core.
Alex: You won't last past the first year, babe.
(They are both laying on the bathroom floor)
Meredith: I don¡Çt know what to say to you.
Izzie: When Dylan died and when the bomb went off did you feel like¡Ä you were moving in slow motion?
Meredith: He was there and then he wasn¡Çt¡Älike I blinked and he was gone.
Izzie: I feel like I'm moving in slow motion and everything around me is moving so fast... and I just want to go back to when things were normal, when I wasn't poor Izzie laying on the bathroom floor in her prom dress with her--her dead fiancée. But I am. So I can't. Am. I'm just stuck... And there's all this pressure cause everyone¡Çs hovering around waiting for me to do something or say something or flip out or yell or cry some more. And I'm happy to play my part. I'm happy to say the lines and do whatever it is I'm supposed to be doing if it would make everyone feel more comfortable. But I don't... I don't know how to do this. I¡ÄI don't know how to be this person. I don't know who this person is.
Meredith: Izzie...
Izzie: How did this happen? Why did we end up here? Why am I alone? Where¡Çs Denny?
Meredith: You're not alone, Izz. (Meredith grabs her hand)
(They both are laying on the bathroom floor)
Cristina: This thing that you¡Çre doing Izzie, it¡Çs not healthy. You gotta take off that dress.
Izzie: Tell me about Shiva.
Cristina: Well, it¡Çs something that you do when someone dies. You know, we did it for my grandmother.
Izzie: How does it work?
Cristina: People bring over food, family comes over it¡Çs supposed to help with the grieving you know, it honors the dead.
Izzie: Sounds nice.
Cristina: Hmm¡Äyeah. Seven days of no other shoes, no work, no sex, not sitting on things higher than a foot, no shaving, no--no (pauses)
Izzie: What?
Cristina: No clean clothes. Izzie, this doesn¡Çt have to be Shiva.
Izzie: You know that they took his body away, and that I¡Çll never see him again. How would you feel if you never saw Burke again?
Bailey (to Denny¡Çs body, which is about to be sent to Memphis for his family): I¡Çm sorry. I¡Çm so sorry.
Callie: I know. He¡Çs in a quarantine because of a thing and I was off-duty and I was worried about him and going crazy and I thought about Izzie so I brought food.
Meredith: For the Shiva.
Callie: Are you gonna let me in?
Meredith: Yeah. (lets her in)
Callie: Relax, I¡Çm not gonna bring it up, you and McDreamy and the sex, I¡Çm not that kinda person.
Meredith (exhales): Good.
Callie: Is it Shiva if Izzie¡Çs Catholic? (
(Finn and Callie are laughing, Meredith walks into the kitchen)
Callie: Oh, Uh, I'm gonna... I'm gonna leave the room for a period of time for no reason whatsoever. (leaves)
Finn: I don¡Çt know what happened last night, between you and Derek.
Meredith: Finn.
Finn: And I don't wanna know, alright, we never said that we are exclusive.
Meredith: But... you have plans.
Finn: Yeah, well I didn't say I wasn't pissed off. I said we weren't exclusive. That¡Çs all I wanted to say... Oh, and this : I know you think you're scary and damaged. It makes you think you don't deserve good things, but you do. And Derek, he's bad for you. But me, I'm a good thing. And if there is a race, if there is a ring, my hat is in.
Alex: How do you not know your kid's pregnant?
Addison: You love your kids, you want to see the best in them.
Alex: Okay, then how do you have a baby and throw it in a trash can?
Addison: Something happens, and you panic. You freeze, and you wanna hide it and pretend like it didn't happen... I get that.
Alex: You get that?
Addison: I do. I just don't get what happens afterward. I don't get how you go back to class and pretend like everything¡Çs fine, everything¡Çs not fine.
Cristina: Don't ever die.
Burke: I'll do my best.
Webber: You didn¡Çt have to make an appointment to see me. I¡Çm your husband.
Adele: Well, an appointment seemed to be the only way I could get your undivided attention because here you are.
Cristina: Why are we making sandwiches again?
Meredith: It¡Çs what you do when someone dies. You cook.
Cristina: Yeah. Yeah, I know what to do when someone dies. I am a Jew. I know food and death, it¡Çs Shiva.
Meredith: Is it Shiva even if she¡Çs Catholic?
Cristina: Well, Shiva¡Çs what I know how to do so¡Ä. yeah, it¡Çs Shiva. And if we¡Çre sending Shiva for Denny, then we should order in because making sandwiches is not the same as cooking.
Meredith: Sandwiches are comfort food. (takes a bite of a sandwich)
Cristina: What is wrong with you?
Meredith (quickly): Everything with me is fine! It's Izzie that we're worried about!
Cristina: Oh, what did you do?
Meredith: Denny died, Burke got shot, let's just have some respective and remember that what I did is a tiny slightly small...
Christina: Whatever, spit it out.
Meredith: I lost my panties last night. (Cristina stares at her)
Cristina: You had sex with Derek in an exam room at the prom?
Meredith: Yes.
Cristina: Last night, you and Shepherd did it. You and McDreamy did the nasty nasty...

(Searching the lockers for food)
Derek: Oh look now here's the motherlode. I got granola, cookies, muffins. Who eats this much?!
George: That's Izzie's locker. She ate a lot so she kept her energy up.
Derek: Eats.

George: Feels a little like she did. She's never coming back here. I don't think she's ever gonna be the same.

Finn: Hello?
Meredith: Hey.

Meredith: Izzie, I gotta go check on Izzie. (leaves)
Callie: Don't take it personally. It's the doctor-thing.

Callie: 4 years of high school. 4 years of college. 4 years of med school, at the time we graduate we're on our late twenties and we haven't done anything except go to school and think about science. Time stops we're socially retarded. I mean look at me, I'm in love with a guy who won't say he loves me back and here I am in his kitchen cooking just hoping he comes home and notices me. I mean I'm a total freak. I'm that girl in the back of the class who eats her hair. Meredith is 17 years old, we're all 17 years old. This is high school with scalpels, Finn.
Meredith: So, what does this mean?
Derek: It means you have a choice. You have a choice to make. And I don't want to rush you into making the decision before you're ready. This morning I was going to come over... I was going to say... Richard: I had a situation, Adele--

(George is on the laying floor face to face with Izzie who is choked up, devastated, and in a daze)
George: (Gently) Maybe... maybe you should... change your clothes. Maybe you want to wear something more comfortable--
Izzie: Stop it!
George: Izzie...
Izzie: Stop it, I mean it! Stop... talking. There is nothing to talk about, do you understand me? There is nothing to discuss.
George: (Whispers ever so softly) I'm so sorry.
